Disconnect problem

Guys, I got a big problem, when im playing I always disconnect in the half of the game,
its getting anoying for clanwars...
What can I do to stop it, I just disconnect in games and get back in the lobby, maybe someone know this problem and can help me

Dont hack and game runs smoothly lol.JK dont really know the problem but its not new so just bare with it and the clan matches thats a different story. I play pub because i get dc'd seconed round in clan matches so its the server id say
