Wide Grenades:
Wide Grenade - Only 1 person owns this. It was from an event, but I don't know the specifics
Wide Grenade-Ares - Obtain from completing collection
Wide Grenade-Chroma - Obtain from completing collection
Wide Grenade-Glorious Phoenix - Obtain from completing collection

Special Grenades (e.g. wider range, damage, function, etc):
Grenade-Phoenix - Obtain from completing collection
Grenade-Rebel - Obtain from completing collection
Smoke-Glorious Phoenix - Win all of the following weapons during a limited time event: AK-47-S-Glorious Phoenix, M4A1-Glorious Phoenix, D.E-Glorious Phoenix
ZM Grenade-Vulcan - Obtain from completing collection

Ranked Grenades:
MK2 Grenade - Ultimate Silver - Obtain from getting gold rank or higher in Ranked Season 2 (Thank you Angel for this!)

Though I just noticed that to complete glorious phoenix collection, the glorious phoenix smoke grenade is needed (which can only be obtained from the event I think).
In which case it is no longer obtainable even with future glorious phoenix crates.

Well, if I recall correctly, the Smoke Phoenix came twice. First time when they released the first 3 Glorious Phoenix, the reward was the Smoke Grenade. The second time when another 3 Glorious Phoenix weapons came out, they brought back the first crates and you could once again obtain Smoke Grenade.

There's also events that lets winners pick a weapon of their choice... For example, Crossfire Film Festival, Make a Wish, Halloween Costume Contest, etc.

I would say if you stick around long enough, everything comes back. I mean, there's a reason people say "nothing is sacred on Crossfire".
