Did i get banned from superreward survey?

how do you figure out if your banned from it... i cant see any surveys...I'd show a print screen but i dont know how to...

To show a print screen:
Go to the super reward survey page you want us to see
Press print screen
Use 'run' to do: mspaint
Paste the picture into Microsoft Paint
Save it and upload it
