Just some advice from someone who had to make surveys for a college as well, you should make your questions more clear. For question 4: Do you skip meal to play online games should be rephrased to have you ever skipped a meal to play online games. Question 7 should be fixed to Have you/do you do poorly in school because of online games. Question 8 needs some sort of time frame, be it a month, year, etc. I'm not sure how critical your teacher/professor will be about these things, but mine deducted points for things like this on the questions.
