Surveys not Functioning

When I try and use a survey, I get to the webpage and there are no drop-down boxes etc for the different survey providers... Why is this?

If anyone knows the answer to this, it'll be much appreciated.

Possibly the cause of a popup blocker. Disable any additional extensions if you are using them.
