Fill out the form as best as you can. Note that you will need to provide 2 out 3 possible proof pieces of evidence to show you own the account before attempting to do any changes to request information about the account.

And since you apparently forgot your registered email and Security Question & Answer you're basically out of luck.

You are responsible to remember and safeguard your account information.
