Happens to me in ghost mode a lot.
Also, it could have been a spectating glitch. (When you die and view teammate, it happens sometimes.)

I think this should be fixed somehow, cause there is a lot of fools who will go "Mortalatis Hacker men! He shoot wif his gunnn!"
