Send a support ticket, I'm not sure what category you'd choose but I think Complaints. You had to do 5 different ZM maps (not just 5 games, each game had to be a different map) on any difficulty with at least two other clan members, but I don't know if the other clan members had to survive to the end with you, I'd imagine that was a requirement but I didn't do it.
