Don't know how to use my Ultimate Game Card

I have a $10 UGC, but I have no idea how to use it >_>
The directions on the back of the card are of no help at all.
This is my time using it, so can someone help me redeem it?

Also, do I have to make a PayByCash account first? It says something about a PlayByCash at step 5..
