Hazard Name Card not showing?

Well the topic says it all. They Hazard Name Card is not showing up when you turn it on.

  I experienced the same thing, won it from some M14 crates and it doesn't work. I turn it on, turn it off, switch to other namecards, the hazard just doesn't work.
