I get same problem occasionally. But it turns out that the wireless connection is not stable enough in ping.
If the computer is put near wireless router without no blocks, ping 192.168.0.1 -n 100 will show the result of <1ms and 1ms. But when there are blocks, the value varies. Currently my test will return most under 10ms. But it can jump to 100+ ms (jittering) in one or 2 line of output. But the game doesn't do the ping test that frequently, and the ping shows to game room will stay in 59, while I think it might be 100+ sometimes.
But I found a strange situation, in some room, my ping is simply very large, 400-2000, while I ping google.com, it shows something like 40. And also, I enter another room it gets 49. And when I thought it stupid to waiting host finish shopping and reenter the lag room, I start to lag up from the beginning. So I quit and join another room, I got normal ping again.
All the rooms are in the same channel and server. So apparently, rooms are not equal -- they might be related to the host. But others apparently have a stable link, even some of them have 200+. I don't if it might be another factor that resulted from this. -
Wait, same problem as in you have normal ping but get no hit reg?I get same problem occasionally. But it turns out that the wireless connection is not stable enough in ping.

I rarely get hit reg with automatic weapons even tho I have a stable 64 ping.
Can take 10+ shots to the chest of a still target before they die.
Its the reason I don't use Automatics much.
With pistols, Snipers, and shotguns I have reg.
With Rifles, MGs, and SMGs I have terrible reg. -

Yes, I think it probably the reason you have normal ping but don't have hit reg. Just because when you think your ping is mini, you actually have a ping jitter when use wireless.
The ping is really a combined factor, I already know several reason you can have a huge ping or fluctuation:
1. Distance (this is basic, play in CF JP or CN would bring you 200+ ping)
2. Traffic in route
3. Cable service is also determined by the neighbour hood network usage, especially now video are so popular and ip tv is also introduced.
4. Your home gateway burden -- simply example, after you start a p2p program (utorrent), it creates lots of sessions and you will experience a slow down.
5. Wireless interference, cordless phone is very efficient in knock you out from a connection when you use 2.4g
6. Computer tasks --- sometimes computer hard drive roaring for something (I guess AV scan or windows update) you game start to lag up, but I guess this is mainly the cpu burden caused issue.
Unregistered hit might related to packet loss, I guess the game use UDP.
