If there is a problem with surveys, most survey providers have an "Offer Report" or a "Check Status" type of page that you can access from the survey provider's page on Z8Games. You can first take it up with them. If that doesn't help [and you're 100% sure that you completed the survey correctly with the correct information], you can send a support ticket by clicking on the orange text, or by clicking on my signature. I'll keep this open if you have any more questions.

Note that nobody on the forum can really help you, not even the moderators.
