how to report

so i caught this dude with good weps and rank hacking. and since he cannot make a new account as of right now, i said myself why not report him? maybe he wont play cf again xD.
now, i have several questions for you guys:

-He was shooting trough wall and 1 hitting everyone, so if i choose 1 shot kill will this cover up the type of hacking he was using or do i have to choose wall hacking?

-"Rounds that Hacking Occurred" - what does this means?

-"Time when Hacking Occurred" - what does this means?

-do i post the link with the replay file from megaupload on the comments or i simply choose the "attach file" option?
