How to ban hackers?

I know where I can ban hackers but I dont know what I have to fill in the box where stays:
Rounds that hacking was found :
I wrote for example in a 5 rounds game, where it stood 5-4 at last, round nr. 8...
Im not sure what I have to fill in really.

can any 1 ban tose nooobs who hak name ghost*cross he use speed hak and others to but sems no1 cares coz they hak all day
