Vertex Lock Failed.

I have this really annoying problem where when I join a room a little box keeps appearing saying "vertex lock failed". Then when I actually join the game I respawn as usual but my screen is black and all I can see is the water that is the the map. Somtimes I see a few fuzzy lines aswell. Can you please help me? Thanks.
