Does anyone else get this error message...?

I've been getting this error message since last week, everyday I get the following errors:

"Disconnected due to incorrect map info." Then I get "Disconnected from server" sometimes I get "Hack tool detected" (WHEN I DON'T HACK) even though I often get kicked and accused of it.

It's always when I'm doing well too!!
I have tried closing all background programs and all that stuff, but it still does it and it's getting on my nerves!

Does anyone know how to fix it?
