disconnected at varous times

main errors:
i can get up to a combination of:

hack tool detected
disconnected from server
disconnected to incorrect map info (all 3 at once)
or
disconnected due to incorrect map info
disconnected from server (2 at once)
or
just the map info

added everything to my exceptions, firewall antivirus e.t.c, still doesn't work, i still crash.
any insight would be a help.
