Happens to me once in a while, though usually it's on Columbia for a split second around crates......I dislike it, but I can't control it, so I just do what I can to live with it.

It's happend also, if u come from A (EE) going to the spawn of GR, sometimes u can see Namecards of ur enemies through the stairs

happens sometimes to me, mostly when someone is crouching behind a wall (head is slightly out of sight) and if I'm aiming at the edge of the wall, a little bit above his/her head. Can't see him/her/it directly, but I do know that there's someone because of the name
